Indications for ELiS Arthroplasty

Symptomatic osteoarthritis of the thumb CMC joint

ELiS arthroplasty is indicated for patients suffering from painful degeneration of the trapeziometacarpal joint.

Eaton–Littler stage II–IV osteoarthritis

Suitable for moderate to advanced stages of CMC joint osteoarthritis where conservative treatment no longer provides sufficient relief. SEE BELLOW

Pain, loss of function, and reduced pinch strength

Indicated for patients experiencing limitations in daily activities due to thumb pain, instability, or decreased grip and pinch strength.

Failure of conservative treatment

Recommended when non-surgical approaches such as splinting, physiotherapy, or injections do not provide lasting improvement.

Eaton I

Early Stage

Eaton stage I CMC osteoarthritis represents the earliest phase with minimal radiographic changes, slight joint space narrowing, and no subchondral sclerosis or osteophyte formation.
Conservative treatment is indicated (orthosis, NSAIDs, corticosteroid injections).

Indications for arthroplasty:
❌ Not indicated. Patients are monitored for disease progression and potential surgical treatment at later stages.

Eaton II

Mild Osteoarthritis

Eaton stage II CMC osteoarthritis is characterized by mild to moderate degenerative changes, minor joint space narrowing, and osteophytes smaller than 2 mm.
Conservative therapy remains first-line, though minimally invasive procedures may be considered in symptomatic cases.

~25% indication for arthroplasty

Indications for arthroplasty:
⚠️ Considered for symptomatic patients not responding to conservative treatment, especially younger and more active individuals seeking to preserve thumb mobility and function.

Eaton III

Advanced Osteoarthritis

Eaton stage III CMC osteoarthritis shows significant joint space narrowing, osteophytes larger than 2 mm, and subchondral sclerosis with deformity of the articular surface.
The ELiS implant is indicated for suitable candidates with good bone quality.

~75% indication for arthroplasty

Indications for arthroplasty:
✅ Strongly indicated. Artroplasty of the CMC joint is preferred over trapeziectomy in active patients who wish to maintain thumb strength and range of motion.

Eaton IV

Generalized Osteoarthritis

Eaton stage IV CMC osteoarthritis involves degenerative changes extending to adjacent joints, especially the scaphotrapeziotrapezoid (STT) complex.
Treatment choice depends on the degree of STT involvement and patient age.

~50% indication for arthroplasty

Indications for arthroplasty:
⚠️ Selective indication. May be combined with other surgical approaches, such as STT arthrodesis, in carefully chosen patients.
